City Health Care Partnership CIC (CHCP CIC) is recruiting for an Urgent Community Response Call Coordinator based at Jean Bishop Integrated Care Centre, Hull. The role supports rapid access to services, acting as the first point of contact for referrals, advice requests and service enquiries within the Frailty team.
