Brownbill Case Management, part of Active Care Group, seeks a Clinical Case Manager to coordinate services for clients with complex health needs in London and Home Counties. You will assess needs, plan interventions and advocate for suitable resources with families and deputies.
Role is full-time, 35 hours per week, hybrid with flexible site arrangements and ongoing professional development. Knowledge of MCA/DoLS and safeguarding is desirable.
