Autonomous Community Nurse – Home Care

City Health Care Partnership CIC is seeking a Community Staff Nurse to provide high-quality nursing care in patients’ homes, enabling independence and safety. You will assess needs, plan care and build trusted relationships with patients, families and health professionals.

You will work autonomously within a supportive community nursing team, delivering person-centred care and contributing to service improvement while developing clinical skills with experienced colleagues.
