Reablement Support Worker – Flexible Hours, Travel

Wiltshire Council is seeking a Reablement Support Worker in Chippenham to help people after hospital discharge, working with occupational therapists to co-develop personalised plans. The role involves visiting customers at home, travelling between visits, and requires a valid UK driving licence and access to a vehicle.

The post is 37 hours per week on a rota basis, with a hybrid working model and a pay rate of £13.92–£14.36 per hour (plus 10% unsocial hours).
