Inpatient Social Worker: Discharge Pathway Lead

Lancashire and South Cumbria NHS Foundation Trust is seeking a Forensic Discharge Pathway Facilitator (Inpatient Social Worker) to join Guild Lodge. The role requires a qualified Social Worker with Social Work England registration and a focus on secure mental health and offender health.

The post holder will work closely with a MDT, lead holistic assessments, and coordinate discharge planning to prevent delays, including liaising with authorities for capacity and funding decisions.
