Overview
In this role you drive community-based care for people with complex psychosis within an assertive outreach pathway. You work in a multidisciplinary team to assess needs, plan robust care, and support recovery in the client’s home and community settings. You’ll manage high-risk cases, engage hard-to-reach service users, and coordinate treatment to maximise quality of life and social inclusion. This role offers meaningful impact by delivering personalized, least-restrictive care in collaboration with service users and their networks.
Responsibilities
- Assess, plan and coordinate care for a defined caseload of service users with complex psychosis
- Deliver ongoing holistic assessments, treatment and support in community settings
- Develop and implement robust care plans aimed at recovery and reduced risk
- Engage with disengaged clients and their support networks to improve engagement and outcomes
- Work within a multidisciplinary team to provide integrated care and treatment
- Utilize a whole-system, community-based approach to maximise independence and social inclusion
- Apply risk assessment, crisis management and CPA process in care delivery
- Promote psychosocial interventions and recovery-focused practice
Key requirements
- Current professional qualification in Mental Health Nursing (live registration)
- Experience in multidisciplinary mental health teams with severe/long-term conditions
- Experience with disengaged clients in community settings
- Skills in risk assessment, crisis management and care delivery
- Knowledge of CPA process and relevant mental health legislation
- Understanding of diverse groups and engaging with them
- Ability to assess, plan, implement and evaluate comprehensive care packages
- Understanding of complex psychosis and impact on service users and carers
