Responsibilities

  • Clinically assess, diagnose, and prescribe where qualified
  • optimise medicines and improve prescribing safety
  • Collaborate with a multidisciplinary team to improve patient outcomes
  • Support medication reviews and medicines optimisation
  • Contribute to service development and innovative working practices
  • Work with Divisional Boards and National Modality Business Support Team to deliver strategic priorities

Key requirements

  • Prior experience in GP practice
  • Excellent communication and interpersonal skills
  • Strong patient-facing consultation ability
  • Attention to detail and numerical/analytical skills
  • Effective teamwork in a multidisciplinary setting
  • Competent in IT and specialist clinical systems
  • Leadership capability and problem-solving
  • Experience in audit or quality improvement projects
  • Strong understanding of medicines optimisation
  • Knowledge of prescription legislation and long-term condition management
  • GPhC registration with no restrictions; Independent Prescriber; PCPEP qualification
