Join our dedicated Ageing Well Team in Wolverhampton as a part-time Qualified Social Worker, contributing 18.5 hours per week to empower and support older adults in our community. Your key responsibilities will include:

• Conducting assessments to identify the needs of older adults and develop personalized care plans.
• Providing direct support and guidance to clients, families, and caregivers.
• Collaborating with multidisciplinary teams to ensure comprehensive service delivery.
• Advocating for clients’ rights and facilitating access to necessary resources and services.
• Monitoring and reviewing care plans to adapt to changing needs and circumstances.

To be successful in this role, you must possess the following essential qualifications and skills:

• A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
• Registration with Social Work England.
• Proven experience working with older adults and knowledge of relevant legislation and best practices.
• Strong communication and interpersonal skills, with the ability to build rapport and trust.
• Excellent organizational skills and the capacity to manage a caseload effectively.
