Practice Consultant, Social Work (Hybrid)

Norfolk County Council is seeking a permanent Practice Consultant to lead social work practice and support hospital discharge. This role is based at the Norfolk and Norwich University Hospital with hybrid working and strong collaboration across health and social care partners.

You will chair meetings, guide complex casework, and coordinate multi-agency safeguarding to ensure safe, timely outcomes for adults. Registration with Social Work England is required.
