Community Nurse Practitioner – Home Care & Case Management

Whittington Hospital NHS Trust is recruiting a Community Nurse Practitioner to manage a caseload within Haringey Hospital at Home and Urgent Community Response Team. The post focuses on preventing hospital admissions and enabling safe early discharge through high-quality home-based care.

You will work autonomously, develop skills in physical assessment, diagnostic reasoning and nurse independent prescribing, and coordinate care across primary and secondary care pathways with MDT input.
