Emergency Dept Social Care Practitioner – Discharge Lead

Portsmouth City Council is seeking a proactive professional to lead the Emergency Department service at Queen Alexandra Hospital. The role supports safe hospital discharges and reduces unnecessary admissions through person-centred, strength-based interventions.

You will work with health and social care colleagues, using digital tools to input data, assess risks, and apply safeguarding principles across Portsmouth. Band 7, £33,119 – £37,562 p.a, UK Right to Work required.
