Adult Social Worker – Hospital Discharge & Out-of-Hours

Sheffield City Council seeks a Qualified Social Worker to join the Adult Hospital First Contact Team at the Northern General Hospital. You will support safe discharges, undertake Care Act assessments, and provide out-of-hours duties across Adult Care and Wellbeing services.
