South London and Maudsley NHS Foundation Trust is seeking a Home Treatment Team Practitioner to join our Older Adults HTT in London. You will deliver crisis resolution and home-based care to adults aged 65+, coordinating with the MDT to provide safe, recovery-focused treatment.
You will gain experience in risk assessment, care planning and engaging with clients in their own homes, with supervision and a personal development plan to support career progression.
