Reablement Care Coordinator – Community Discharge Specialist

Northern Care Alliance NHS Foundation Trust’s Home First Reablement Service in Salford is recruiting caring professionals to support people at home, promote recovery and safe discharge, and coordinate reablement plans across community teams.
