We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult’s Social Work Hospital Team in Hammersmith & Fulham. This role is pivotal in providing support and intervention for adults facing a range of social, emotional, and practical challenges within a hospital setting. You will work collaboratively with healthcare professionals, patients, and their families to ensure the best outcomes for individuals in need of social care services.
Key responsibilities include:
- Conducting assessments of patients’ social care needs and determining appropriate interventions.
- Developing and implementing care plans in collaboration with multidisciplinary teams.
- Providing emotional support and guidance to patients and their families during hospital stays.
- Advocating for patients’ rights and ensuring their voices are heard in care decisions.
- Facilitating discharge planning and connecting patients with community resources.
To be considered for this role, you must possess the following essential qualifications and skills:
- A recognized qualification in Social Work (e.g., BA or MA in Social Work).
- Registration with Social Work England.
- Experience in adult social work, particularly within a hospital or healthcare setting.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent problem-solving abilities and a proactive approach to case management.
