Recovery‑Focused Health & Wellbeing Practitioner

The Forward Trust is seeking a Health and Wellbeing Practitioner to provide recovery-oriented services at HMP Bronzefield. You will manage a caseload, deliver both one-to-one and group interventions, and work with a multidisciplinary team to support adults with substance misuse and mental health needs.

You will carry out assessments, develop SMART care plans, and maintain high-quality records while coordinating with community services for post-release continuity.
