Senior Occupational Therapist – Discharge & Rapid Response

Derbyshire Community Health Services NHS Foundation Trust is seeking an experienced occupational therapist to work on Alton Ward at Clay Cross Hospital, supporting discharge planning and rehabilitation within a community hospital setting.

You will carry a clinical caseload, provide supervision to junior staff, and collaborate with nursing, physiotherapy and social care colleagues to coordinate safe, timely discharges and patient-centred care across the locality.
