Senior Case Manager: Lead Clinician & Mentor (Hybrid)

Active Care Group in the Wigan area is seeking a Senior Case Manager to lead a complex caseload while mentoring other Case Managers. You will coordinate rehabilitation plans, ensure evidence-based practice, and maintain high-quality documentation.
