Job Description:
West London NHS Trust is seeking a Band 6 Senior Community Nurse/District Nurse to join our Greenford locality team, driving community care excellence. You will lead a multi-disciplinary group, manage caseloads with GPs and ICS partners, and champion patient-centred strategies to support independence at home.
We require a Registered Nurse with a health-related graduate qualification or a Specialist Practitioner in District Nursing, plus post-registration community experience and a valid car.
