Practice Nurse: Chronic Disease & Diabetes Care (34 hrs)

Preston Grove Medical Centre is seeking an enthusiastic Practice Nurse to join our nursing team. You will contribute to high-quality patient care and work with our Nurse Manager, other Practice Nurses, and GPAs to deliver chronic disease management and preventive care.

The role involves independent practice within a supportive team, with a focus on diabetes care, wound management, immunisations, and health promotion. This is a permanent, full-time post at 34 hours per week.
