Job Description:
Oldham’s Integrated Discharge Team is seeking an experienced Social Worker to support patients being discharged from Royal Oldham Hospital under the D2A process. You will carry out assessments, develop care plans, and coordinate multi-agency support to enable safe, timely discharges.
The role requires a Social Work degree and SW England registration, with responsibilities including safeguarding, best interest decisions, and advocacy for residents to live well and independently.
