Senior Social Worker – Discharge & Care Leadership

Hull City Council seeks a Hospital Senior Social Worker (Grade 9) for a permanent full-time post, potentially based in hospital or community teams. The role covers 8am–8pm shifts within a 7-day pattern to assess individuals post discharge and support independence.

You will operate within an integrated multi-disciplinary team, driving person-centred discharge plans and contributing to the new Discharge to Assess model, with registration required with Social Worker England.
