Stroke Recovery Practitioner – Community Rehab Team

Nottingham CityCare Partnership CIC is recruiting a full-time Band 4 Stroke Practitioner for the Community Stroke Service in Nottingham. You will assess and support rehabilitation in patients’ homes, care homes and clinics as part of a large multidisciplinary team, managing your own caseload and contributing to audits and service development.
