Short-Term Pathways Case Officer – Community Care

Kent County Council is seeking a Case Officer in the East Kent area to work with adults over 18 who recently hospitalised. You will assess needs, plan care and arrange ongoing support to promote independence, with collaboration across health and partner agencies.
