Innovative Health Visitor – Community Health Leader

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Health Visitor to join the Health Visiting Service. You will lead delivery of the Healthy Child Programme, focusing on the 6 High Impact Areas, using evidence-based practice and strong teamwork.

The role centres on assessing families, developing personalised care plans, and collaborating with partners to support children and young people across our communities. UK driving licence is required.
