Community Mental Health Nurse — Lead Recovery & Home Visits

Oxford Health NHS Foundation Trust is seeking a passionate Psychiatric Nurse to join the Oak Team within City and North East Oxfordshire AMHT. You will work in a multi-disciplinary setting delivering holistic, person-centred care across GP-aligned localities and partner services.

The role involves bio-psycho-social assessments, brief interventions, treatment planning with service users and carers, and risk management. Travel across Trust sites and home visits are required.
