Home Dialysis RN: Educator, Coach & Care Leader

Fresenius Medical Care is seeking an experienced Home Therapies RN CAP 2 to educate and manage patients for peritoneal and home hemodialysis. You will train patients and partners in a clinic setting and during initial home visits, coordinating care with the team.

As an RN, you will assess learning needs, adjust treatment plans, and lead safety-focused care. Occasional travel to patient homes, on-call rotations, and supervision of staff are included in this role.
