NELFT NHS Foundation Trust is seeking a Bridging Service Care Assistant to support the Bridging Care and Rehabilitation Service in the community. This Band 3 role provides short-term homebased care and rehabilitation, working under supervision and coordinating with care teams.
You will facilitate client independence using a reablement approach, plan daily rosters, assist with medication administration where trained, and maintain accurate records for audits.
