Social Worker Assistant — Neuro Care & Community Support

Jacobs Neurological Centre is seeking a Social Worker Assistant to support assessments and social arrangements for adults with neurological conditions. You will liaise with families, external agencies, and the care team to progress outcomes and promote independence.

The role requires understanding of the Mental Capacity Act and safeguarding, plus strong communication skills. 37.5 hours weekly, with training opportunities available through Elysium Healthcare.
