Community Nursing Caseload Lead

Wirral Community Health And Care NHS Foundation Trust is seeking a Community Nursing Caseload Manager (Band 6) for 35 hours per week, fixed term for 6 months. You will manage planned programmes of care for patients in domiciliary and clinic settings, and assess, plan, implement and evaluate nursing care at the community/primary care interface.

Applicants should have district nursing qualifications, current NMC registration, and evidence of ongoing professional development.
