Senior Community Nurse – Learning Disabilities

Livewell Southwest is seeking a Community Nurse to support adults with learning disabilities across Plymouth. The role covers assessment, planning and review of a caseload with potential physical and mental health needs in a person-centred approach.

You will work within a MDT, coordinating with GP practices, hospital services and voluntary agencies, and contribute to safeguarding, health promotion and education for staff and carers.
