Discharge-Focused Mental Health Social Worker

Leeds City Council is seeking a Social Worker (Mental Health Hospital Discharge) to support safe, person-centred discharge from inpatient mental health services. The role is based at The Mount Hospital with hybrid working and regular ward reviews.

You will manage a varied caseload, undertake Care Act assessments, coordinate discharge plans and work with NHS colleagues, families and carers to promote independence and recovery.
