Community Care Officer: Empower Independence & Support

Nottinghamshire County Council is seeking a Community Care Officer to join the Adult Social Care team. You will assess needs, plan personalised support, and link people with community resources to promote independence.

You will work with Social Workers and health colleagues, handle a caseload, and contribute to safeguarding. A relevant health or social care qualification (NVQ Level 3 or equivalent) is required, plus IT skills and at least two years in care work.
