Rapid Response Therapy Support Worker

Derbyshire Community Health Services NHS Foundation Trust is looking for a Rehabilitation Support Worker to join our Rapid Response Team, delivering rehabilitation alongside the therapy group in community settings. You will support patients at home and in care homes, creating patient-led care plans and contributing to discharge planning with the multidisciplinary team.

A weekend roster to respond to rapid therapy visits is included.
