Job Description:
The Leeds Teaching Hospital NHS Trust is recruiting a CNS Team Leader to oversee care for 16–25-year-olds transitioning from paediatric to adult renal services. The post is based at St James’s University Hospital with strong links to the paediatric team at LGI and satellite dialysis units across West Yorkshire.
You will ensure seamless care transitions, coordinate MDT meetings, and support young adults with their chronic conditions, balancing workload with a patient-centered approach.
