Care Act Assessor — Community Social Care (Contract)

Midlands Partnership NHS Foundation Trust is recruiting a Band 4 Care Act Assessor to cover maternity leave within Stafford’s Adult Social Care team. The role involves holistic Care Act assessments, home visits, and close collaboration with the older persons with physical disability team.

You will use the Care Act 2014 to determine eligibility and outcomes, manage your own caseload, and work with multidisciplinary colleagues. A driving license and access to a vehicle are essential.
