Job Description:
Hertfordshire County Council is seeking a Hospital Team Manager in Stevenage to lead a fast-paced Transfer of Care Hub. The role oversees a seven-day discharge service (08:00–18:00, weekends included), with one full weekend off every eight weeks and time off in lieu where applicable.
The post holder will provide operational leadership, ensure person-centred practice, and support performance management across hospital sites and community settings.
