Job Description:
St George’s, Epsom and St Helier University Hospitals and Health Group is recruiting to provide high-quality clinical assessment, intervention and care coordination for care home residents with frailty and complex health needs.
The post holder will act as the clinical delivery arm of the multidisciplinary Care Home Support Team, translating MDT decisions into timely hands-on care within the care home, with emphasis on personalised Urgent Care Plans and appropriate community services.
