Working Pattern: Hybrid
Hours: Monday-Friday, initially 9am-5pm, with some 10am-6pm shifts. Later moving to Monday-Sunday, 1 in 6 rota

Eden Brown Synergy is currently recruiting for a qualified Occupational Therapist to join the Integrated Discharge Service (IDS) within Islington Adult Social Services.
This is an exciting opportunity to join a dynamic and fast-paced Hospital Discharge Service, supporting Islington residents to return home safely while promoting independence, recovery and positive outcomes.
The Role
As an Occupational Therapist, you will play a key role in assessing risk and designing interventions to support residents at different stages of the hospital discharge pathway.
- Supporting the Discharge to Assess pathway through referral screening and discharge planning.
- Participating in the 7-day screening rota, expected to commence after approximately 2 months.
- Completing post-discharge assessments and providing one-off therapy interventions.
- Assessing home environments and arranging equipment and minor adaptations.
- Providing advice on managing daily activities and manual handling.
- Developing Reablement Plans and Goals using a strengths-based, outcome-focused approach.
- Working closely with Social Workers, Reablement, Health colleagues and wider multidisciplinary teams.
- Supporting independence, recovery and admission prevention.
What We’re Looking For
- Qualified Occupational Therapist.
- Relevant professional registration.
- Experience in adult social care, hospital discharge, community or reablement settings is desirable.
- Strong assessment, risk management and clinical decision‑making skills.
- A person‑centred and strengths‑based approach.
- Ability to work effectively within a multidisciplinary team.
- Excellent communication and organisational skills.
