SEND Key Worker: Remote/Flexible EHCP Planning & Support

Tripod Partners is seeking a SEND Key Worker to undertake statutory assessments and develop and maintain Education, Health and Care Plans (EHCP) across Cheshire East Council. You will manage a caseload of Children with SEND, attend annual reviews and modify plans as required, while liaising with childcare, education and vocational providers to source suitable placements for young people with EHCPs.
