As a vital member of our team, you will play a crucial role in assessing and responding to the needs of adults in our community. Your expertise will help ensure that individuals receive the support and services they require to enhance their well‑being and independence.
Key responsibilities include:
- Conducting comprehensive assessments of individuals’ needs and circumstances.
- Developing and implementing tailored support plans in collaboration with service users and their families.
- Providing information and advice to service users regarding available resources and services.
- Working closely with multidisciplinary teams and external agencies to coordinate care and support.
- Maintaining accurate and up‑to‑date case records in compliance with legal and organizational standards.
To be considered for this role, you must possess:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
- Registration with Social Work England.
- Proven experience in adult social work, particularly in assessment and safeguarding.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent organizational skills and the ability to manage a caseload effectively.
