Senior Care Lead: Dementia-Care Champion

HC-One is seeking a Senior Care Assistant in England to lead a care team and deliver person-centered care for residents across dementia, nursing, residential and specialist settings.

You will plan and implement care plans, maintain accurate records, support residents and their families, and work closely with the Registered Nurse and Care Manager. A Level 2 Diploma in Adult Social Care (or progression to Level 3) is preferred.
