Youth Mental Health Practitioner – Home-Based MDT Leader

Leeds Children’s and Young People’s Mental Health Service, part of Leeds Community Healthcare NHS Trust, is seeking an enthusiastic practitioner to join the CORE team. The role involves working with children, young people and families in home and clinic settings as part of a busy multidisciplinary service.

You will deliver evidence-based mental health interventions, participate in MDT reviews, triage referrals, and support leadership within a dynamic care pathway for young people up to 18.
