Active Care Group is seeking a Complex Care Specialist Support Worker to join a regional response team across England and Wales. You’ll help set up and stabilise complex care packages, support hospital discharges and provide person-centred care to clients in their own homes.
Expect travel within your region, paid mileage, accommodation when working away and a £3,000 annual car allowance, with an overall salary up to £29,000 depending on experience.
