Proactive Social Prescribing Lead: Development & Coaching

York CVS is seeking a Social Prescribing Manager to lead the Proactive Social Prescribing Service in York. You will line manage the team, drive development and monitor performance to meet KPIs.

This fixed-term role runs until 31 March 2027, involves working across locations, and requires a degree or equivalent experience in health or social care.
