Community Social Care Assistant Home Visits & PACE Training

Oxford Health in Buckinghamshire is seeking a part-time Social Work Assistant for the Community Team. You will visit people in their homes and in placements across the south of Buckinghamshire, input directly into LAS, and train to carry out the Appropriate Adult function under PACE on a rota basis.

You will assess and review needs, develop high quality assessments, plans and reviews focusing on independence and outcomes, and work in partnership with other professionals and organisations.
