To implement assessed care needs and help to maintain progress in rehabilitation / reablement. To assist patients / individuals with personal care tasks, which is delegated as part of the reablement care plan as well as any other additional patient care duties. This will include assistance with meals and drink preparation if required. To provide and assist with the care of people in their homes, working with individuals and their families to ensure a person-centred approach including the participation in clinical care procedures e.g., recording observations and application of topical cream To support admission prevention by responding to incidents of carer breakdown, provide rapid support for individuals to prevent admission to hospital.
