Senior OT – Mental Health & Home Rehab Specialist

West London NHS Trust is seeking a Band 6 Occupational Therapist to join the team at Meridian Ward, Hammersmith & Fulham Mental Health Unit. You will assess and treat clients admitted to the older adult inpatient ward, applying evidence-based OT practice in home and hospital settings.

The role involves comprehensive assessments, cognitive rehabilitation, and discharge planning with a focus on client-centred care and governance frameworks. Significant collaboration with MDT colleagues is expected.
