Hospital Social Worker – Discharge & Care Planning Pro

Charles Hunter Associates is recruiting a Social Worker for Merseyside to help facilitate safe hospital discharges and develop care packages. The role requires assessing risk factors and coordinating with the multidisciplinary team to ensure timely, safe patient outcomes.
