Neonatal Discharge & Family Care Navigator

The Neonatal Discharge Planner role at Sandwell and West Birmingham NHS Trust will establish and maintain an effective communication system to improve health and safeguarding for neonates who have stayed in the neonatal unit at Midland Metropolitan University Hospital.

You will work with a multi-disciplinary team to support families, promote family integrated care and facilitate safe, well-coordinated discharge plans, bridging hospital and community care for babies and their families.
